Call The Ice Center at 682-3615 to request a Registration Form. We'll send it to you by mail.
Or drop by The Ice Center to pick one up by visiting our main desk at the Ice Center.
Email the Ice Center to request a Registration Form. We'll send it to you by mail. Make sure you put "Registration Form" in the subject line of your email.
(Note: Print-quality PDF file will appear pixelated or fuzzy on-screen but will print clearly)
